Participant Agreement and Program Metrics

As part of our commitment to transparency and accountability, the Rockingham Community Care Program tracks and reports specific metrics to demonstrate the effectiveness of our services. This is essential for fulfilling the requirements of our grant from the State of New Hampshire and the New Hampshire Department of Health and Human Services. By participating in our program, you agree to allow us to collect and report on certain data, as well as use your photos and stories in our outreach efforts.

Metrics Tracked

To measure the success and impact of our program, we will track the following metrics:

– Financial Assistance Provided: The amount and type of financial assistance distributed, including housing, utilities, medical expenses, and groceries.

– Beneficiary Demographics: Information on the age, gender, and socioeconomic status of program participants to ensure we are reaching those in need.

– Service Utilization: Data on the frequency and types of services used by participants, such as the number of housing or utility bills paid.

– Health and Well-Being Indicators: Metrics related to improvements in participants’ health and well-being, such as reductions in medical debt or increased access to healthcare services.

– Participant Feedback: Regular surveys and feedback forms to gather insights on participant satisfaction and areas for improvement.

Use of Photos and Stories

In addition to tracking metrics, we may request to use photos and personal stories from our participants to help promote the Rockingham Community Care Program. This may include sharing photos and stories on our social media platforms to highlight the impact of our services and raise awareness. We may also feature participant stories and images in brochures, reports, newsletters, and other promotional materials to illustrate the program’s benefits and successes.

By enrolling in the Rockingham Community Care Program, you consent to:

– The use of your photos and personal stories in our social media posts and sales collateral, should you choose to participate in these promotional activities.*
